SHIPPING INSTRUCTIONS

To send us a case, please include the following items and please observe the OSHA guidelines: 

• Prescription (must be separate from impressions and outside of the plastic bag to avoid possible contamination)
• Impression (must be kept in plastic bags)
• Opposing arch model and bite registration to facilitate the articulation process (models should be wrapped in bubble wrap if possible to avoid breakage)

Kelly Dental Lab provides complimentary shipping of your case to our lab via one-way UPS.  Most doctors use our convenient boxes and labels to ship cases to us, and are billed for returned shipment.  If you need additional boxes or labels please call our office.

Tulsa Doctors – We provide pick-up service in the Tulsa area.  For pick up, please call Kelly Dental Lab courier, Delivery Service, at 918-748-5553 when you have items to be picked up from your office.  Calls received before 1 p.m. will be picked up and delivered to the lab by 5 p.m. the same day.  Calls received after 1 p.m. will be picked up and delivered to the lab the next day.
